Senior Backend Engineer - Clinical Automations (RPA)
Asterix Health
Job Description
We’re hiring a Senior Backend Engineer, Clinical Automation at Asterix
There aren’t enough doctors in the UK, and the system can no longer keep up. Millions of patients wait weeks for appointments, and preventable conditions go unnoticed.
Asterix is building a new model of care where highly qualified doctors are augmented by AI, safely, consistently and measurably. Our clinical platform, DoctorOS, makes doctors up to 10x more productive. We’re backed by Entrepreneurs First, recently closed an oversubscribed funding round, and are live with some of the UK’s largest healthcare providers.
The problem you’ll solve
Clinical AI is only useful if it can see the patient’s record and act inside the systems GP practices already use: EMIS Web, SystmOne and Docman. Most have no modern API that does what we need.
Today we bridge that gap with RPA bots, workflow automation and FHIR integrations that pull worklists, lab results and documents into DoctorOS. It’s live across multiple practices, it works, and it has the most room of anything in our stack to become excellent. Right now it has no dedicated owner.
This is a founding-level role. You’ll own the whole path end to end, from the bots that operate clinical systems to the pipelines and backend services that make it reliable, auditable and safe. You’ll also shape what comes next, moving from low-code RPA towards code-first automation and official integrations such as IM1.
What you’ll own:
- Clinical system automation: make our bots deterministic, idempotent and observable, decide where UiPath still fits and where code-first tools (Python, TypeScript, Playwright) serve us better, and lead that migration
- Backend and data pipelines:
build the queues, APIs and sync workers on AWS and Supabase, designed for retries, duplicates, partial failures and backlogs, and own orchestration (n8n today)
- Infrastructure and security: own the Windows VMs, HSCN connectivity, VPNs and secrets, lock them down properly, and build monitoring that catches problems before a clinician does
- Clinical safety:
work with our clinical safety team to meet NHS standards including DCB0129/DCB0160 and DSPT, and document how each automation behaves and fails
- Collaboration: partner with doctors, operations, product and AI engineers to find what to automate next, and turn tribal knowledge into runbooks and standards
Who you are
We don’t care about pedigree. We care about what you’ve built and how you think.
- Strong production backend experience in Python
- Experience designing and owning APIs and backend services
- Solid grasp of data modelling, async systems and reliability
- Good judgement around performance and safety
- Comfortable working across frontend, product and clinical teams
Nice to have: experience with LLM APIs, regulated environments, founding teams, or React/TypeScript.
You care whether the doctor has the right data at 8am, not just whether the bot ran. You know when a bot is the answer, when an API is, and when to delete an automation entirely. And when a VPN drops or a clinical system changes overnight, you stay calm, fix it and stop it happening again.
How we work
We’re humble, assuming we’re wrong until the data or a clinician proves otherwise. We have a founder mentality and don’t wait for permission. We take the work seriously but not ourselves. And we work in person in London because it makes us faster and better.
To be upfront: this isn’t a 9-to-5, and we’ll sometimes ask for that little bit extra. We work hard, but with intention.
What’s on offer:
- £80k–£100k+ base + generous equity
- Bupa private health and dental
- All equipment covered
- Onsite in Shoreditch, London
If you want to make clinical AI work in the real world, not just in a demo, this is it.
